CONDITIONS AND PROCEDURE FOR NEW STUDENTS REGISTRATION Welcome to Tumaini University Makumira! The following are the conditions for registration. Only those students who meet ALL of these conditions will be registered. Requirements: Original certificates: Form IV, VI, ( and diploma/Degree where applicable) Original Payment receipts/Bank slips One recent passport photograph. Valid Health Insurance Card (for those who posses it) Medical examination form – duly filled in by a physician for those who have not submitted one. (Available in the application form at page 5. Seehttp://makumira.ac.tz/index.php/downloads/application-form) Signing the Declaration Form Procedure for registration: Pick the Registration and Declaration forms from the secretary in front of the Administration block. Fill the forms as required. See the secretaries of the respective faculties for scrutiny of the certificates.
